Anniversary Announcement

Instructions

Announcements are published for couples celebrating their 25th anniversary, and for every five years thereafter (30th, 35th, 40th, etc.).

Simply complete this form, attach your image in electronic format and click "I agree and submit."

If you would rather print out a form or need to submit a printed photo, please return to the menu above and choose the PDF version. You can then print that out and mail it to or drop it off at The Daily Record office, 212 E. Liberty St., PO Box 918, Wooster, OH 44691.

If you mail your photo or drop it off, include a self-addressed, stamped envelope to have your photo returned by mail or, after the article has appeared, come to The Daily Record office Monday through Friday, 8 a.m.-5 p.m., to pick it up.

Announcements are printed on Tuesdays, space permitting, on a first-come, first-served basis. The Daily Record reserves the right to edit copy as necessary. All completed forms must contain the name and phone number of a contact person.

I do hereby declare and warrant that I am the copyright owner (i) because I took the photograph and authored the text, film, print, slide, artwork, digital image, or other material (the "Materials") which I am submitting for publication, or (ii) I obtained express, written permission from the photographer and any other author or creator of the Materials to submit them to HolmesCountyShopper for publication.

I hereby grant a non-exclusive license to HolmesCountyShopper to publish these Materials and distribute them in any of the company’s associated sites, publications or other media, and to create derivative works. I represent that I have the legal authority to grant such a license. I agree to indemnify and hold harmless HolmesCountyShopper and its related companies, employees and agents from any and all liability, damages and expenses (including reasonable attorneys’ fees) that may be incurred as a result of publishing these Materials, including any claims brought by any other person claiming an interest in the Materials or their subject matter.

I understand that the Materials may be cropped or modified at the sole discretion of the site owner. I further understand that the Materials may be rejected for any reason and are not guaranteed to be published."
